Homemaker and Personal Care Services
Our team will design a personalized care program that provides the support, supervision, and services necessary to maximize the health and welfare of an individual. This holistic approach to personal care includes tasks that increase the independence of the individual within their home or community. With services aimed at both support and maintenance of their unique environment, we’ll develop a care plan that’s based on their physical and mental condition.
Homemaker Personal Care (HPC) Transportation
With the goal of helping individuals get where they need to go, Homemaker Personal Care Transportation is the perfect solution for accessing community resources, activities, and services. HPC Transportation is available in addition to medical transportation services.
Non-Medical Transportation
Transportation used to get to, from, or between:
- Adult day support, career support, or vocational habilitation
- Volunteer activities
- Post-secondary educational programs including internships
- Drop-off or transfer locations (As specified in Ohio Administrative Code 5123-9-18 (B)(16)(a) to (B)(16)(e) of this rule.)
Adult Day Activity Support (Coming Soon!)
- Adult Day Activity Support is designed to help individuals acquire skills, engage with the community, boost independence, and expand personal choice. Taking place in a non-residential setting, regularly scheduled activities help improve adaptive skills, socialization, and self-help.
- Each activity is designed to keep pace with the individual’s interests, goals, and outcomes. They will have the opportunity to participate in community activities and become part of a thriving network.
- Access to a caring support network that fosters and maintains social skill development. Individuals will experience independence and personal growth through participation and building relationships with community involvement.
- In addition to increased opportunities to build problem-solving skills, individuals can take part in community living. Communication, mobility, dressing, dining, hygiene, and dressing are each involved in growing community living skills.
- Since practice makes perfect, behavioral support strategies play a key role in reinforcing newly learned skills. Communication systems, mobility devices, and other activities help the individual grow and thrive through continued participation in community life. They will learn self-advocacy skills, skills that foster greater independence, and how to take responsibility over the services they receive. Soon enough, they will become integrated members of their community while staying both productive and independent.

Our person-centered service plans are therapeutic in nature, with health-related guidance that caters to the needs of the individual.
